I recommend you build all bucks prior to laying blocks to
expedite construction.

I use a combination of both illustrations. I rip the lumber to
butt up to the inside of the outside form edge and to the outside of
the inside form edge, with a 2x3 spreader on the inside only. I then
staple sill gasket or 6 mil poly around the buck and then add double
headed nails for the concrete to hold the buck. This method keeps
the integrity of the insulation on the outside and still allows a
nailing edge on the inside.

Consider using masonry unit windows. Then simply glue foam to the
icf to act as a bulkhead, after the pour just slip a saw between and
remove the foam, attach the masonry unit, and instead of using an
extension jamb one can apply sheetrock in the opening with either a
square or bullnose corner application where it meets the wall
shetrock. Nice look in many contemporary homes where a wide wood
trim is not part of the design criteria.
